I think we are indeed seeing JFK in a sitting position, propped up by Jackie, heading under the Triple Underpass. It seems odd but makes sense if you think of it.
When Jackie went to sit back down in her seat, it would have been difficult, as JFK, lying on his side, would have taken up the entire back seat. Jackie would likely have had a fair amount of adrenaline in her system by this point and setting JFK upright would not have been difficult. JFK's back brace would have kept his torso rigid and aided her in this task.
There are, however, two things that bother me:
1. Why is his head not flopping forward, backward or to one side?
2. Where is the great blowout flap on the right side of his skull, so visible earler in the Z film and in autopsy photos?
